1. BOOK your appointment time. Select an appointment time, fill out your information, and then click place order.

2. COMPLETE screening questionnaire. Click on Take the Screening Questionnaire and answer the questions.

A QR code will be emailed to you at completion. You MUST have a QR code when you arrive at your appointment.

When you arrive at our pharmacy for your COVID test, please drive around to the back of the store and call us at (608)839-3335 from outside. A staff member will come out to you with the COVID test.
